Supporting Documents

   

To complete your DBS application, we must confirm your identity. You will need to bring your original documents to one of our local Trust Services offices to do this. You can find the locations and contact details for all our Trust Services offices here.

Please ensure that you’re aware of the following when bringing in your documents:

  • Documents need to be valid, current, and original – we can’t accept photocopies.
  • Printed documents are not acceptable. For example, internet bank statements or online versions of utility bills.
  • At least one document must confirm your date of birth and at least one document needs to confirm your current address.
  • You must provide a full address history covering the last five years (with no gaps) and all previous names you have had on the DBS application form.
  • Documents proving any name changes must be provided to link your documents together.
    • For example, you provide your birth certificate, but your driving license is in your new name. You must provide your marriage/civil partnership certificate, adoption certificate, or deed poll document to prove your name change.

We must verify your original documents and take copies before completing your application. You will need to visit one of our local Trust Services offices to do this. You can find the locations and contact details for all our Trust Services offices here.

We can only send your application to the DBS Authority once we've received your online form and supporting documents.

Sadly, we can’t ask the DBS Authority to speed up the processing of your certificate. As an employer, NHS Professionals can only follow up on any delay with the DBS Authority if your application takes more than 60 days to clear any individual DBS checking stages.

You will not be able to work if your previous DBS has expired until your new DBS application is cleared.
