Band 5 – pay rate £15.40ph Basic. £20.02 Saturday/Nights; £24.64 Sunday/Bank Holidays

The post holder will;

  • Work within the NMC Code of Professional Conduct.
  • Assess, plan, implement and evaluate packages of patient care under the direction of the District Nurse team leader.
  • Evaluate and reassess nursing care according to clinical need, identifying problems against set goals.
  • Be responsible for admitting and discharging patients from the caseload.
  • Supervise care provided by more junior members of staff.
  • Delegate nursing care appropriately to more junior staff.
  • Adhere to policies, procedures and guidelines.
  • Report back to District Nurse Team Leader/Deputy Team Leader as appropriate.
  • Work closely with all Primary Care team members and others concerned with patient care.
  • Supervise other members of the team where appropriate.
  • Assist the District Nurse Team Leader in undertaking recruitment and selection procedures within the team.
  • Assist the District Nurse Team Leader in the undertaking of annual Staff Appraisal and Development Reviews with team members.
  • Support all members of the team, supervise and teach students, junior staff, patients and carers.
  • Be aware of budgetary constraints and effective resource management.
  • Adhere to the organisation’s policies and procedures.
  • Ensure that local and national Health and Safety regulations are adhered to by all members of the team.
  • Be responsible for the identification and reporting of any risks, including those identified by any member of the team, and reporting them to the District Nurse Team Leader.
  • Liaise with the District Nurse Team Leader/Senior Managers where appropriate.
  • Attend appropriate meetings.
  • Take an active part in clinical supervision and reflective practice sessions.
  • Assist the District Nurse Team Leader in ensuring that the allocation of staff, duty rotas and annual leave entitlement is sufficient to meet service needs.
  • Assist in service audits where indicated.
  • Assist in an organised and planned induction of new staff in the District Nurse Team.
  • Assist the District Nurse Team Leader in maintaining effective communication systems with all members of the District Nurse Team, and within the organisation, other agencies and the voluntary sector.
  • Maintain records in accordance with the organisation’s policy and that of the NMC Guidelines for records and record keeping.
  • Maintain evidence based care in personal and team practice.
  • Take part in research if appropriate.
  • Participate in educational programmes for staff within the District Nurse Team and learners placed in the team, and complete assessment forms as appropriate.
  • Keep abreast of own personal and professional development.
  • Will regularly be required to carry out nursing care, which includes the moving and handling of patients with varying disabilities, and the handling of equipment.
  • Required to travel throughout the day and work alone for frequent periods.
  • Required to work in a variety of settings, including peoples’ homes.
  • Required to frequently work with body fluids.


In order to apply for a post you must have a minimum of 6 months experience in a similar role held within the last 2 years . Hold NMC Registered Nurse (RN1) qualification.
