We have a placement opportunity available for a CPN at Lambeth Hospital




  • To carry out comprehensive individual assessments, of clients, physical, psychological, mental health and social situation. This information then forms the basis on which an individualised recovery plan is formulated for each client. This process needs to be done in partnership with clients, their carers and other partner agencies involved in their care. The planning will be for the whole client journey and may include completion of CCA's and follow up on aftercare plans.


  • To be responsible for the clinical care of a defined caseload of clients on an individual basis with complex drug/alcohol related problems ensuring clinical care is delivered in accordance with Trust policies, national guidance and evidence based practice


  • To be involved in the development and monitoring of standards of care, clinical audit, research and outcomes to enhance and develop good clinical practice.


  • Attend meetings and case conferences as appropriate with client, carers and other professionals, this includes child protection conferences, CPA's, Panel meetings, multi-agency meetings etc. Where necessary, reports would need to be supplied.


  • To regularly review Recovery plans with clients and ensure that goals are SMART and cover all relevant domains of treatment. To ensure that all activity and information is entered in the clinical note system, relevant service databases and within time frames specified. To comply fully with all NTDMS and trust data requirements.


  • To administer vaccinations, injections and undertake BBV testing. To deliver other physical health interventions as relevant to client’s needs.


  • If appropriately qualified to deliver non-medical prescribing within scope of practice and trust guidelines for alcohol and drug clients attending the service.


  • To develop positive working relationships and advocate for clients with a wide range of statutory and voluntary organisations to include GPs, Community Mental Health Teams, General Hospitals, criminal justice system and appropriate others. To be involved in the development and implementation of new practice, policies and guidelines.


  • To mentor and supervise junior staff/students/ peer mentors where appropriate.


  • To lead on coordinating satellite clinics and developing specialist areas, service user involvement, policies and guidance as required.


  • To participate and cover specific duty/triage sessions. To be involved in the day to day running of the service ensuring that all clients’ needs are met. To cover for senior staff where necessary. Post-holder may be asked to carry out tasks in other areas if there are pressures on other parts of the Treatment system.


Skills and Personal Qualities:

  • To apply for this post, you will have the following:
  • Delivery of a high-quality service, working both independently and as part of a well-established team. 
  • You should be a good team player, preferably have experience and an interest in community work, and if possible, experience of working within adult or older people’s service.
  • You will engage, assess and formulate appropriate plans of care for service users, coordinate their care and signpost to other services as appropriate, as well as being a good communicator who can maintain good working relationships.
  • Each Community Psychiatric Nurse will be required to hold care co-ordinator responsibility for several service users

    Essential Criteria:


  • Current Professional Qualification in Mental Health Nursing with a live NMC registration
  • 6 months experience in community Mental Health 
  • Able to build constructive relationships with clients and carers using warmth and empathy, using good communication skills.
