We have a requirement for a Band 6 CPN120 to work part time 3 days a week (Monday - Friday) 9am-5pm, based in SE16.

Job description:

Skills, knowledge, experience required:

             Experience and understanding of working in a community setting and with a multi-disciplinary team for at least 5 years.

             Working with service users with a severe and enduring mental illness. 

             The post holder will be an autonomous practitioner who will support a safe transition of patients discharged from secondary to Primary care, with a focus on enabling GP practices to provide effective, continuing care and to ensure access to support in terms of wider social and social inclusion/self-management needs.

             Specialist knowledge and application of risk assessments, interventions and outcome measures, relevant to clients with enduring mental illness.

             To provide appropriate short-term interventions for clients with more complex and severe, enduring mental illnesses (SMI) in primary care with the aim of preventing deterioration, relapse and (re-) admission to secondary care mental health services.

             Ability to undertake comprehensive Assessment and care planning to ensure a whole person approach, involving others in assessment as necessary.

             Ability to develop professional networks and keep abreast of local resources; third sector partners, primary care, housing, education, social care and other statutory organisations in order to ensure that the service user is able to navigate the full health and social care system.

             To monitor; risk, progress and where necessary compliance including the service users views to their care and treatment and when appropriate to work in partnership with the service users family and carers.

             To explain and discuss medication with the service user, including side effects they may experience and the role that the medication may have in their recovery or on going care

             Communication skills (verbally and in writing) 

             Ability to promote the empowerment of individual service users to manage their health as much as possible and signpost use of Self Directed Support / direct payments as indicated.

             Understanding of diversity issues, and working in culturally diverse areas

             Understanding of the supervisory process

             Awareness of current developments, e.g. recovery model, personalisation.

             Ability to organise and respond efficiently to complex information
